Individual sessions:	30 minutes: $ 60.00 45 minutes: $100.00 60 minutes: $125.00
Couples sessions:	$125.00

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking your insurance carrier the following questions:

• Do I have mental health insurance benefits?
• Is this provider on the provider list?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, checks or credit cards are accepted for payment.

Cancellation Policy
If you are unable to make your scheduled appointment, please be courteous to call and reschedule or cancel in order for that time slot to be available for others.
